Setting Up Forms

Purpose

Use this procedure to specify the Image Type, Record Type, and Sub Type that will be assigned to an appraisal form when it is scanned. The barcode that is on the form indicates the type of form that is being scanned, and the PACS master record to which the form is to be attached.

The following forms are supported for scanning in PACS:

Procedure

  1. In PACS Appraisal, choose Tools > Document Imaging Maintenance > Forms Maintenance.

    The Form Maintenance dialog box is displayed.

  2. Click New Type.

    The Forms Maintenance Details dialog box is displayed.

  3. In the Form field, complete the following fields:
